I will give some examples.
Quick time events (QTE)
Looks like you are watching a nice FMV then all of a sudden icons appear on screen that you have to press.
Unskipable cutscenes
You just watched 20-30 seconds of boss man talking shit to you now its time to kick his arse.... oops you died now you have to watch the same cutscene again.
Escort mission
You know protectign an NPC while you take them to their destination whilst hundreds of enemies shoot at the NPC and not you.

Non-pauseable (is that how you write it?) cutscenes. Like, WTF? Games should allow us to pause any cutscene, not just skip them with Start. Can't say how many times I've been pissing myself out of fearing that Start would skip the whole thing.

I don't mind QTEs, but the other two suck. There's a few really long pre-boss scenes towards the end of FFX, which I had to go through plenty of times when first playing the game. Good thing they were good scenes though! And good thing I know how to beat each boss now. lol
And escort missions make me worry, instead of having fun.

Definitely. I've never understood those sort of decisions. Start button should pause the cutscene with the option to skip! I don't see why you aren't just given that choice.
Not so much an annoyance nowadays, but back when WW2 was the in fps setting... I almost always hated tank missions... they were just annoying...
Stupid button layouts, some just don't make sense.
But I think the cutscene thing may be the most annoying. It's weird though, it's the sort of thing I can't think of any until I come across them in the game..

I don't mind QTEs and sometimes they can be pretty cool (a la God of War). What I hate is when you have to watch a cutscene every time you restart and can't skip it.
I hate when games don't have good checkpoints...not that I mind the challenge, just that nowadays the games are so boring that you feel like giving up after one try if the points aren't placed well. I love Dark Soul because it's challenging and checkpoints are hard to find but the game is fun so I don't mind that it's extremely tough.
I hate games that don't go in details with realism for the sake of making it easier and accessible for newbs. I dislike Japanese-style games that are full of useless dialog, clicks and menus.
I pretty much hate all games in general. Sometimes you find games like ICO, SotC and Demon Souls that are out of this world and amazingly immersive but generally they're all a bunch of bologna.

Wow I'm totally the opposite there. I wish that game had less QTEs, especially since they were so annoying and completely unfair at points. At the same time, I think having QTEs to beat those bosses and minibosses pretty much one of the reasons why it made GoW enjoyable, to my knowledge, I think it was the first time QTEs were used in such a cool way. Other reasons were the epic boss battles and the easy-to-follow storyline which was actually well done.
The gameplay otherwise was pretty mediocre. Kinda repetitive. Just go around and kill everyone...

No QTEs in God of War would just be...weird. They go hand in hand with the epic death scenes of bosses and shit, if they got rid of them what would you do? Just stand there continuing to spam buttons further? That would get old (older than QTEs) fast imo. That or they'd get replaced by a cutscene
Apart from that I'd have to go with Soldier95b's list, I hate not being able to pause in cutscenes and not being able to customise controls

QTE's in Shenmue were awesome. Especially the boxing game. I think they're ok, sometimes they're can work well... but sometimes they're just annoying.

This strays a little from the subject, but I dislike how a LOT of games feel the need to have a 'look down the barrel of the gun aiming mode' just because its popular is COD doesn't mean that various 3rd person shooters need it. it just feels tacky
Also +1 for cutscenes that you can't pause and if you do they skip.. and ones you can't skip! So annoying
Every cutscene should pause when you press Start and have the option to skip or resume

alot of my hates have been mentioned already such as unpausable+unskippable cutscenes.
Other mechanics I hate:
Level Caps
I enjoy being rewarded so once I hit the level cap in a game my interest in the game drops by about 75% XD.
Games like Fallout 3 annoyed me for their incredibly low level cap and killed alot of my motivation to play and explore the rest of the world.
Whats wrong with giving the player the choice of being bad ass if they put the hours in to the game. To tackle balance issue they could scale the story missions up accordingly with the level. Skyrim did a better job on this with a larger level cap and good scaling where it was needed without sacrificing the accomplishment of being powerful.
FF13 featured level caps but alteast you could increase it by continuing the story, still, they should have left the option to continue leveling up further in there for players who wanted to.
Lack of a New game +
All games should feature some form of new game+ whether its just a "harder" difficulty setting or a complete new game+ mode with bonus content and the ability to carry over items, experience from previous save file.
One big offender for this is ff13 since the battle system doesnt completely unlock until the last hours of the game, they could have added a new game + where the battle system is unlocked and the tutorials removed. Eternal Sonata's encore mode was pretty cool as the battle system was fully unlocked for you from the start (party levels).
