    Well, everyone is working with multi million polygon models now as work flows are mostly about creating high quality asset and then the LoD models and baking information from hi to low poly into textures. (albedo, normal, occlusion, subsurface, displacement (either height or vector) etc..)

    What gamers see is the low poly model with possibility of tesselated displacement making additional polygons when close. (and additional lower polygon models when they are further away.)
