    I think people have movie scenarios in mind, where you have the big bad evil villain who has nothing left to lose and decides to $#@! everything as much as he can before his little empire topples. Even a real world scenario where a leader was completely bat-$#@! insane and willing to do that, the order has to go through several people and security measures to even be done. It's not like the 'Launch the Nukes' button is on the leader's desk. It just seems unrealistic they'd be used even in desperation.

    I think the only real-world threat of nuclear weapons is terrorist cells using them. That is a real and terrifying prospect. Or, worse, a secretly aided cell by a competing nation.

    Well, you can't rule out the prospect of false flag attacks though. The only nation that doesn't have 100% security affirmation and guarantee of its nuclear stockpile is Pakistan. But even this is a stretch to say the Pakistanis would allow them to get into the hands of some terrorist cell for use on another country.

    Far more likely is the use of a nuke by the government on its own citizens in dirty bomb fashion or detonation as a false flag maneuver to bring the nation into a war that otherwise wouldn't have happened in the first place.
