The Fallout 4 PS5 upgrade is out now as a freebie for existing owners of the game, but those of you who claimed Fallout 4 on PS Plus currently are not able to claim the update free.
Oddly enough it appears that Xbox Game Pass owners are able to claim the Fallout 4 Xbox Series X/S upgrade, but PS5 owners can only claim it free if the have outright purchased Fallout 4 on PS4 and not simply obtained the game through PS Plus.

PS5 Upgrade should be available for you if you OWN Fallout 4 on the PSN account. Still looking into this for the PS+ users!
