You can find the Traveler's Grisly Trophy in God of War at Realm Tears. These portals to another dimension spawn some tough enemies, so get geared up!
You're looking for a certain enemy type called Travelers. These powerful creatures spawn from Realm Tears and will drop this item for you. You will bump into other Travelers throughout your mission, but it's well worth seeing out the 18 Realm Tears to stock up!
